Agreement to These Terms

By booking an appointment, using our website, or receiving services at Forever Young Skin Clinic, you agree to these Terms of Service. If you do not agree, please do not use our services.

Our Services

Forever Young Skin Clinic offers professional esthetician services including, but not limited to, facials, chemical peels, extractions, waxing, and other skin treatments. All services are performed by a licensed esthetician in the state of Connecticut.

We reserve the right to modify, discontinue, or add services at any time.

Health Disclosure and Client Responsibility

Your safety is our priority. Before receiving any treatment, you are required to:

• Complete an accurate and complete health intake form

• Disclose all known skin conditions, allergies, sensitivities, and relevant medical history

• Inform us of all prescription medications, over the counter products, and supplements you use

• Notify us of any changes to your health between appointments

You are responsible for providing accurate information. We are not liable for adverse reactions caused by incomplete or inaccurate disclosures.

Certain medical conditions, medications (such as Accutane, blood thinners, or recent Botox), or recent procedures may make some treatments unsafe for you. We reserve the right to decline or modify any service based on your health history, at our professional discretion.

If you have a medical skin condition, we strongly encourage you to consult a licensed dermatologist or physician before booking.

Appointments

Booking: Appointments may be booked by phone, email, or through our online booking system if available.

Confirmation: You will receive a confirmation of your appointment. Please review appointment details carefully.

Arriving late: If you arrive more than 15 minutes late, we may need to shorten or reschedule your service to accommodate other clients. Full payment may still apply.

Cancellation and No Show Policy

We request at least 24 hours notice for cancellations or rescheduling.

• Cancellations with less than 24 hours notice may be subject to a cancellation fee of up to 50% of the scheduled service price.

• No shows without notice may be charged the full service price.

• We understand emergencies happen. We will consider exceptions on a case by case basis.

Repeat cancellations or no shows may require a deposit to book future appointments.

Refund Policy

We do not offer refunds on services already rendered. If you are dissatisfied with a service, please contact us within 72 hours and we will work with you to address your concerns.

Gift cards and prepaid packages are non-refundable but may be transferred to another person.

Payment

Payment is due at the time of service. We accept cash, major credit cards, and debit cards. Prices are listed on our website and are subject to change without notice.

Minors

Clients under 18 years of age must have a parent or legal guardian present during the service and sign a consent form. We do not provide services to clients under 13 years of age.

Before and After Photography

We may request to take before and after photos for our records or for educational and marketing purposes. Photography for marketing purposes requires your separate written consent. You may decline without affecting the services you receive.

Scope of Practice

Forever Young Skin Clinic provides esthetician services only. We do not diagnose, treat, or cure any medical condition. Our services are not a substitute for medical care. If you have concerns about a skin condition, we encourage you to see a licensed physician or dermatologist.

Nothing we say during a consultation or service should be interpreted as medical advice.

Limitation of Liability

To the fullest extent permitted by Connecticut law, Forever Young Skin Clinic's liability for any claim arising from our services or your use of our website is limited to the amount you paid for the specific service in question.

We are not liable for:

• Reactions caused by undisclosed health conditions, medications, or allergies

• Results that vary from your expectations when services were performed correctly

• Any indirect, incidental, or consequential damages

Esthetic services carry inherent risks, including temporary redness, sensitivity, or, in rare cases, irritation. By booking a service, you acknowledge these risks.

We make no warranties that our website will be error free or uninterrupted.

Governing Law

These Terms are governed by the laws of the State of Connecticut. Any disputes will be resolved in the courts of New Haven County, Connecticut.

We may update these Terms of Service at any time. Updates will be posted on our website. Continued use of our services after any update means you accept the revised terms.
